HBR is now participating in the Food Service Program through December 2020. What does that mean? Just a friendly reminder that physicals and vaccinations are due by October 15th, 2020 if you have not already submitted them. This includes remote learners. Every child over the age of 11 will need to have a TDap and Meningococcal vaccination or a current signed religious objection waiver. These are requirements for all 6th-grade students. Dental exams will not be due until May 15th, 2021. Please make sure you sign on the parent signature line of the physical form. Incomplete exams cannot be processed. Physicals also have a parent section with yes/no questions that need to be circled on the top of the second page. If you are unable to get a date prior to the October 15th deadline, you will need to submit a proof of appointment letter from the provider.
