PHYSICAL AND IMMUNIZATION REQUIREMENTS

Immunization/Physical Information for the 2022-2023 School Year

The State of Illinois requires that a current physical be on file AT THE SCHOOL by the first day of school for Kindergarten, 6th, and 9th graders. This physical must be performed by a licensed physician or provider and be documented on the State of Illinois Certificate of Child Health Examination form. This form is available for download below and should also be available at your physician's office.

Required immunizations MUST BE COMPLETED before October 15th of the school year or the child WILL NOT be able to continue to attend school beginning on October 15th. The student will be excluded from school until the appropriate immunizations are administered and the proper certifications have been submitted to the school.

A current physical is one that has been performed by a physician or provider within 1 year from the first day of school and must be completed on the State of Illinois Certificate of Child Health Examination form dated 11/2015, not Sports Physical Forms. Physicals turned in of an outdated form will not be accepted. The health history portion at the top of the second page must be completed and signed by the child's parent or legal guardian. The Illinois State Law requires all students entering 6th grade to have 1 dose of meningococcal vaccine as well as TDap vaccine. Doses must be received after the child's 11th birthday. For all students entering 12th grade, the second dose of the meningococcal vaccine is required unless the first dose was given after the age of 16.

If your child will be participating in sports, please be aware that his/her physical must cover the entire time frame in which that sport is practiced and played. We recommend that your child have his/her physical done during the summer months so that he/she will be able to participate in all practices and events in any/all sports.
